Job summary

Northern Illinois Hospice in Rockford, IL is seeking a compassionate Hospice Social Worker (PRN) to bring comfort and dignity to patients and families. You will collaborate with nurses and the care team to support emotional well-being and guide families through the hospice journey.

Requirements include a Master’s in Social Work (MSW), IL LSW or LCSW preferred, 1+ year healthcare or hospice experience, strong communication and empathy, and a valid driver’s license.

Qualifications

• Illinois LSW or LCSW preferred.
• 1+ year experience as a Social Worker (healthcare or hospice preferred).
• Strong communication, empathy, and organizational skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Complete psychosocial assessments in hospice care.
• Develop care plans and provide counseling as a Social Worker.
• Coordinate community resources and referrals.
• Support advance care planning (DPOA-HC).
• Maintain timely, accurate documentation.
• Collaborate with the interdisciplinary hospice team.
• Ensure compliance with Medicare and Illinois standards.

A Day in the Life of a Hospice Social Worker (PRN)

As a Hospice Social Worker, your PRN shifts are focused on connection and compassion. You’ll meet with patients and families, provide emotional support, and guide them through the hospice journey. This Social Worker role includes collaborating with nurses and the care team, advocating for patient needs, and ensuring comfort during critical moments. Every Hospice Social Worker brings calm, clarity, and care when it matters most.
