Job summary

Elara Caring is seeking a Medical Social Worker (MSW) to join our home health team in the United States. You will assess patients’ psychosocial, financial, environmental, and safety needs, develop care plans, and coordinate with physicians and the care team to support positive outcomes.

You will connect patients and families with community resources, counsel on social and emotional needs, and facilitate planning for safe discharges.

Qualifications

• Current State License as a Social Worker.
• Master’s or Doctoral Degree in Social Work from a CSWE-accredited school.
• 1+ year of social work experience in a healthcare setting.
• Ability to lift up to 50 pounds and travel between patient homes.

Responsibilities

As a Medical Social Worker (MSW), you’ll contribute to our success in the following ways:

What is Required?

• Current State License as a Social Worker
• Master’s Degree or Doctoral Degree in Social Work from a school of Social Work accredited by the Council of Social Work Education
• 1+ year of social work experience in a healthcare setting
• Social work skills as defined as general social work standards
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
