Job summary

Elara Caring is actively seeking a Medical Social Worker to join our team serving the Williams & Fulton County area. You will assess patients, complete evaluations, develop care plans, and coordinate with physicians to ensure the right care at the right time in the right place.

Qualifications

• Must hold a current State License as a Social Worker and a Master’s or Doctoral Degree in Social Work from an CSWE-accredited program.
• Minimum 1+ year of social work experience in a healthcare setting.
• Ability to frequently lift, push, pull, and move up to 50 pounds; positioning or transferring patients and moving equipment.

• Complete initial social work evaluations and reassess patient needs during follow-up visits.
• Develop and update individualized care plans based on patient needs, goals, and physician orders.

• Counsel and educate patients and families to address social, emotional, and care-related needs.
• Coordinate with the care team to support continuity of care and discharge planning.

What is Required?

• Current State License as a Social Worker Master's Degree or Doctoral Degree in Social Work from a school of Social Work accredited by the Council of Social Work Education 1+ year of social work experience in a healthcare setting Social work skills as defined as general social work standards Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
