Job summary

The Care Team Hospice is seeking a licensed Medical Social Worker (MSW) to support patients and families in home or facility settings. You will provide psychosocial assessments, counseling, and advance care planning while coordinating with an interdisciplinary team to ensure holistic, patient-centered care.

Requires MSW (preferred) or BSW with supervision; hospice or home health experience is valued. On-call rotation may be required with additional compensation.

Qualifications

• Master's Degree in Social Work preferred.
• Bachelor's Degree in Social Work may be accepted with required supervision per state regulations.
• Experience in hospice, home health, healthcare, or case management preferred.
