About Us

The founders of Day City Hospice leveraged their experience from corporate America to create a patient-centered hospice that maximizes resources at the bedside while being operated locally. Our mission is to provide compassionate hospice care rooted in the communities we serve. When end-of-life care is necessary, patients and their families are in their most vulnerable time. Our Ohio-based management team, through our local clinicians, responds promptly and appropriately to our patients' needs because we are local.

Schedule

PRN hours between 8 am - 5 pm, Monday - Friday

Responsibilities

• Perform Licensed Massage Therapy assessments, diagnostic tests, skilled procedures, and ongoing evaluations for clients receiving services under a medically approved Plan of Care.
• Develop a massage therapy care plan to guide other health team members involved in the plan of care. Review and update plans according to agency policy and client needs.
• Document all findings, plans, interventions, and outcomes, and submit documentation to the hospice office promptly.
• Communicate plans and changes to physicians, the client's Clinical Team Lead, and other caregivers through the plan of care, progress notes, and care conferences.
• Report schedule changes and physician orders to the office on the day they occur.
• Provide exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to maintain this level of service.

Qualifications

• Graduate of a Licensed Massage Therapy Program.
• Current license/registration as a Licensed Massage Therapist.
• Minimum of two (2) years’ experience in the healthcare field.
• Hospice experience preferred.
• Licensed driver with insured vehicle in accordance with state and organizational requirements, in good working order.
• Ability to organize and prioritize workload independently.
