Job summary

Concho Hearts Hospice is seeking a proactive Hospice Marketer / Community Liaison to expand its presence across Pasadena, TX and nearby areas. You’ll build relationships with physicians, hospitals, and community partners, educate on hospice services, and drive referrals with an ethical, patient-first focus.

The role emphasizes travel within the assigned territory, collaboration with clinical teams, and a commitment to growing awareness of hospice services in the local healthcare community.

Qualifications

• Experience developing relationships with physicians, hospitals, and healthcare facilities.
• Strong communication, networking, and relationship-building skills.
• Self-motivated, organized, outgoing, and results-driven.
• Comfortable managing an assigned territory independently.
• Ability to identify opportunities and consistently follow through.

Responsibilities

• Build and maintain relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other healthcare referral partners.
• Identify new referral opportunities and develop strategies to increase hospice awareness and appropriate admissions.
• Represent Concho Hearts Hospice throughout Pasadena and surrounding communities.
• Educate healthcare professionals and community partners about hospice services and eligibility.
• Conduct regular visits and follow-ups with referral sources.
• Develop and manage a consistent marketing schedule and territory plan.
• Collaborate with clinical and leadership teams to support a smooth referral and admission experience.
• Participate in networking events, community outreach, and educational presentations.
• Track referral activity and communicate marketing progress.
• Work toward established growth goals while maintaining ethical and compliant marketing practices.
