One Stop Hospice Inc is seeking a compassionate Hospice Licensed Vocational Nurse (LVN) to provide hands-on nursing care to terminally ill patients. Job Summary:
The Hospice Licensed Vocational Nurse (LVN) provides compassionate, hands-on nursing care to terminally ill patients under the supervision of a Registered Nurse (RN) and in accordance with the patient’s plan of care. The LVN supports symptom and pain management, promotes comfort, and offers emotional support to patients and their families in their homes or residential facilities.
Key Responsibilities:
Provide direct patient care including monitoring vital signs, administering medications, and assisting with personal hygiene and comfort measures.
Report changes in patient condition promptly to the RN case manager or physician.
Document care and observations accurately and promptly in the electronic medical record (EMR).
Assist in managing symptoms such as pain, shortness of breath, nausea, or agitation according to the care plan.
Educate and support patients and families regarding the disease process, hospice philosophy, and what to expect during the end-of-life process.
Communicate effectively with the interdisciplinary team including RNs, physicians, social workers, chaplains, and aides.
Maintain compliance with all hospice policies, procedures, and applicable federal and state regulations.
Participate in on-call rotation, evening/weekend shifts, and emergency visits as assigned.
Qualifications:
Licensure: Active and unrestricted Licensed Vocational Nurse (LVN) license in the state of practice.
Education: Graduate of an accredited LVN/LPN program.
Experience: Minimum of 1 year of nursing experience preferred; hospice or palliative care experience strongly desired.
Certifications: Current CPR certification; valid driver’s license and reliable transportation.
Skills: Strong clinical and interpersonal skills, compassionate bedside manner, and ability to work independently with minimal supervision.
Technology: Experience with EMR systems preferred; basic computer literacy required.
Working Conditions:
Travels to patients’ homes or residential facilities (mileage reimbursement typically provided).
Exposed to various home settings and patient conditions, including emotional stress related to end-of-life care.
Must be available for flexible scheduling including weekends, holidays, and on-call shifts as needed.
