Job summary

Choice Health At Home is seeking an Intake Coordinator (LVN) for its San Antonio hospice branch. This full-time role involves managing initial patient referrals, verifying insurance benefits, obtaining authorizations, and accurately entering data into EMR.

The ideal candidate has 1 year of referral intake experience and LVN/LPN preferred. The position requires a Texas LVN license, a valid driver’s license, and familiarity with HomeCare HomeBase.

Qualifications

• Minimum one year of experience in referral intake is preferred.
• Experience with third party payers, Medicare and Medicaid highly desirable.
• Medical field background preferred, including familiarity with medical terminology.
• Excellent computer and customer service skills.
• LVN/LPN preferred.
• Valid driver’s license and valid auto insurance.
• HomeCare HomeBase experience preferred.

Responsibilities

• Receives and appropriately documents new referrals in a timely manner.
• Verifies each patient’s payor source and maintains and tracks insurance authorizations.
• Coordinates care with Nursing Supervisors, patients, and the interdisciplinary group.
• Provides effective communication to patients, staff members, other health care professionals and referral sources.
• Responds to Agency and patient needs in a professional and creative manner.
• Demonstrates commitment, professional growth and competency.
• Participates in Agency-sponsored in-service training.
• Participates in the Agency’s QAPI program.
• Promotes the Agency philosophy and administrative policies to ensure quality of care.
