Qualifications

  • Must possess a current active LVN license verified by CA Board.
  • Minimum one year nursing experience required; ideally with end-of-life patients.
  • Current CPR certification is mandatory.

Base pay range

$37.97/hr - $42.00/hr

Hospice of the East Bay (HEB) is a community-based hospice and palliative care program providing services in the home and at our six-bed Hospice facility for terminally ill patients and those in need of palliative care. Care is given to patients and families by an interdisciplinary group. Bereavement care is provided to the family for a minimum of one year after the death. Hospice and palliative care offer a sponsored care program to provide services regardless of patient’s ability to pay. Position Summary

With general direction from the Hospice Physician (or Nurse Practitioner) and under the supervision of a Registered Nurse (RN), the Licensed Vocational Nurse (LVN) provides direct nursing care and related assistance to patients and families as required by the assigned program/department. Although the LVN reports to a primary manager, HEB expects the LVN may work in more than one program or department.

Essential Functions

  • Performs all patient care duties allowed by an LVN determined by clinical judgment skills or under the direction of the RN.
  • Maintains communication with patient, family and facility staff as required and including education on pain/symptom management and psycho-social aspects of hospice care.
  • May attend, as requested, and present appropriate patient-related information at interdisciplinary team meetings.
  • Gathers data for plan of care and implementation of the interdisciplinary group care plan for each patient.
  • Observes patients and reports changes in condition to appropriate person (e.g., Case Manager).
  • Communicates with appropriate medical staff regarding patient acuity, scheduling, and pending physician communication.
  • Completes documentation timely and accurately.
  • Attends and participates in nursing staff meetings and HEB in-services.
  • Participates in the agency quality improvement activities as assigned.
  • Performs other duties as required.

Position Requirements

  • Ability to establish and maintain cooperative relationships with professionals and families
  • Ability to collaboratively work as part of an interdisciplinary group
  • Willingness and flexibility to be cross-trained in more than one HEB program or department
  • Communicates daily with Nurse Case Manager, Team Managers and appropriate interdisciplinary group members via phone or voice mail system as appropriate regarding changes in patient’s condition and care plan
  • Coordinate cares with other agencies, suppliers and facilities participating in patient care in conjunction with Nurse Case Manager

Experience & Education

  • Current, active LVN license required; must be verified by the CA Board of Vocational Nursing and Psychiatric Technicians
  • At least one year nursing experience as LVN required; experience working in acute care and/or with end-of-life patients highly desirable
  • Current CA Driver’s License and car in good working condition required
  • Current CPR certification required
  • IV certification preferred
