Qualifications

• Current state license as an LVN.

• Current BLS, hands-on CPR certification.
• Valid driver’s license and reliable, insured transportation.

Responsibilities

• Implements current nursing practice following a comprehensive assessment.
• Provide accurate and timely documentation of patient services.
• Assess and provide patient education relevant to diagnosis.
• Participates in coordination of hospice services.
• Uses and prepares equipment and supplies effectively.
• Provides appropriate pain/symptom management.
