Qualifications

• Must be licensed in the agency state of operation.
• At least one year of nursing experience.
• Knowledge of hospice care and role on an interdisciplinary care team.

Responsibilities

• Provides technical care to patients in the home setting, receiving assignments and reporting, and administering prescribed medications under RN supervision.
• Observes, records, and reports changes in patient condition and documents them in the clinical record.
• Performs prescribed procedures as instructed and supervised by an RN.
• Participates in IDT team planning and interdisciplinary care.
• Assists home health aides and physicians/RN as needed during procedures.
• Utilizes electronic medical record documentation tools accurately.
• Participates in on-call rotation as scheduled.
