Job summary

Tranquility Hospice is seeking a licensed Practical Nurse to provide hospice nursing services under the direction of the RN/Case Manager. The role involves caring for patients, documenting care, and educating families throughout the care journey.

Shifts include 2nd and 3rd Saturdays 2 PM–8 AM and Sundays 2 PM–8 AM. Mileage reimbursement, auto allowance, and 401K benefits are provided to support staff.

Qualifications

• Must be graduated from an accredited School of Nursing.
• Must be licensed in Ohio as a Licensed Practical Nurse.
• One year of LPN experience; hospice experience preferred.

Responsibilities

• Assist the RN/Case Manager with providing hospice care.
• Submit complete and accurate clinical notes in a timely manner.
• Assist in educating patients and families about the dying process.
• Use Electronic Health Records, Email, computers, and cell phone devices.
