Job summary

The Visiting Nurse Association of Southeast Missouri is seeking an LPN to provide skilled hospice care in patients' homes and other settings in Missouri. You will follow physician orders, document in the EMR, and communicate changes to the RN staff and the IDG team.

Responsibilities include medication reconciliation, participating in IDG meetings, taking on-call rotations, and ensuring the plan of care aligns with CMS regulations and state guidelines for hospice/home health nursing.

Responsibilities

• Provide skilled care to patients under agency policies, physician orders, and CMS regulations for Hospice.
• Make pertinent nursing observations and communicate with RN staff, TCM, Hospice physician and IDG team; document promptly.
• Participate in IDG meetings and document IDG notes.
• Take on-call rotation as scheduled.
• Maintain current clinical notes in EMR reflecting plan of care and care provided.
• Inform doctor of changes in patient conditions with narrative documentation.
• Review medication profile and reconcile medications weekly.
• Assist the RN in performing specialized duties.
• Report identified needs or problems of patients or families to the RN promptly.
• Follow the plan of care as initiated by RN/physician.
• Comply with state guidelines and scope of practice for LPN in homecare/hospice.
• Provide accurate information to patients/families as outlined in the care plan.
