Visiting Nurse Association of Southeast Missouri is seeking an LPN to provide skilled hospice nursing in patients' homes under physician orders and CMS guidelines.

You will perform nursing observations, document care, participate in IDG meetings, take on-call rotations, reconcile medications weekly, and follow the established care plan.

Training includes a 30-day onboarding and computer training for the handheld device used during visits.

Responsibilities

• Provide skilled hospice nursing in patients' homes under physician orders and CMS guidelines.
• Perform nursing observations and document care.
• Participate in IDG meetings and doc IDG notes.
• Take on-call rotation as scheduled.
• Reconcile medications weekly and update the care plan.
• Inform changes in patient's condition to RN/physician with narrative notes.
