Croí Health is seeking an experienced Hospice LPN to work 40 hours per week across the South Shore. You will collaborate with the RN case manager to deliver skilled nursing at patients' homes and report changes to physicians and the care team.

You must have MA LPN license, 2+ years in LPN and hospice, and prefer homecare/hospice experience. Strong assessment, communication, and the ability to work in a team while also functioning independently are essential.

Qualifikationen

• Massachusetts LPN license in good standing required.
• 2+ years of LPN experience required.
• 2+ years of Hospice experience required.
• Prior homecare and hospice experience preferred.
• Strong clinical assessment skills and the ability to prioritize and triage patients.
• Exceptional communication skills and the ability to work within a team structure.
• Ability to work independently and access resources for patients.

• Provide skilled nursing services to patients in their homes in collaboration with the RN case manager.
• Communicate changes in patient status directly to the physician and case manager.
• Deliver direct patient care and participate in a team-based care model.
