As an LPN, you will provide direct patient care under RN supervision, follow the plan of care, document services accurately, educate patients and families, and collaborate with the interdisciplinary team to achieve positive patient outcomes in

Qualifications

• Graduate of an accredited LPN/LVN program and licensed in Iowa.
• Three years of nursing experience; hospice or medical/surgical preferred.
• Current driver's license and insured vehicle as required.
• Excellent verbal and written communication skills.
• Understanding of hospice philosophy and end-of-life care.

Responsibilities

• Provide direct patient care under the supervision of a registered nurse per plan of care.
• Implement treatments and coordinate with the interdisciplinary team to meet patient needs.
• Provide accurate and timely documentation of patient services reflecting the plan of care.
• Educate patients and families on diagnosis and self-plan of care.
• Coordinate hospice services and report needs to the team (OT, PT, SW, etc.).
• Prepare and use equipment adhering to aseptic technique.
• Assist with pain and symptom management and patient comfort.
