Qualifications

• Graduate of an accredited school of practical/nursing (LPN).
• Current state LPN license must be active.
• Two years of LPN experience, preferably in home health setting.
• Proof of CPR certification and Hepatitis profile.

Responsibilities

• Administer skilled nursing care under RN supervision for clients in their homes.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
