Job summary

Enhabit Home Health & Hospice is seeking a licensed nurse to administer skilled nursing services in patients' homes under physician orders and the established plan of care. You will educate patients and families, collaborate with the health care team, and ensure care aligns with the POC.

Applicants must be a graduate of an approved nursing program, licensed in the practicing state, with CPR certification and validDriver's license.

Qualifications

• Graduate of an approved school of clinical education.
• Licensed in the state where practicing.
• Minimum one year of licensed nursing experience.
• Basic technology skills including operation of a mobile device.
• Medicare home health or hospice experience preferred.

Responsibilities

• Administer skilled nursing services to patients per physician orders and the plan of care (POC).
• Educate the patient, family and health care team as part of care delivery.
• Perform services in accordance with the physician’s orders and the POC.

Responsibilities

Administer skilled nursing services to patients requiring intermittent professional services. Teach the patient, family, and other members of the health care team. Perform services in accordance with the physician’s orders and the established plan of care (POC).

Qualifications

• Must be a graduate of an approved school of clinical education. Must be licensed in the state where they currently practice. Must have at least one year experience as a licensed nurse.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Education and experience, preferred

• Experience as a licensed professional may be deferred with a review and approval from the regional vice president for operations or regional director.
• Medicare home health or hospice experience is preferred.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification
