Job summary

Hudson Valley Hospice is seeking a licensed practical nurse to provide compassionate homecare support under the supervision of an RN or physician. The role emphasizes patient education, data collection, and collaboration with the interdisciplinary team across care settings.

Full-time position with day shifts and on-call rotation. NY LPN license and minimum two years of nursing experience preferred, with Hospice certification pursued after 500 hours or within the first year of employment.

Qualifications

• New York State licensed as a practical nurse.
• Minimum of two years’ experience in nursing – homecare or hospice setting desirable.
• Hospice & Palliative Licensed Nurse Certification after 500 hours or 1 year of employment.

Responsibilities

• Provide services within State Practice Act boundaries and Department of Patient Services responsibilities.
• Support team decisions and hospice plan of care as directed by the RN Case Manager.
• Observe and collect data to identify needs and plan care.
• Provide patient/family education and guidance per interdisciplinary plan of care.
• Record patient information (I&O, VS) and document progress.
• Educate and document patient progress for nurses and physicians.
