St. Joseph Hospice is seeking a full-time Licensed Practical Nurse (LPN) in Monroe, LA. The role involves delivering nursing and supportive care under the guidance of an RN within the Interdisciplinary Plan of Care.

Qualifications

• Current Louisiana Licensed Practical Nurse (LPN) license required.
• Minimum of 2-3 years as a Licensed Practical Nurse (LPN).
• Current CPR certification required.
• Have dependable transportation, driver’s license, and car insurance.
• Previous Hospice experience preferred.

Responsibilities

• Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
• Communicates with the attending physicians, other hospice team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.
• Document all services provided and submit them on appropriate forms.
• Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits.
• Report changes in patient condition and needs to RN.
