Job summary

Hillcrest Health & Living is seeking a compassionate Hospice LPN for Hillcrest Hospice in Lincoln, NE. You will provide direct patient care, support symptom management, and communicate with families and care teams to ensure dignified, comfort-focused end-of-life care.

Ideal candidates have 3+ years of nursing experience, with 2+ years in hospice/home care favored. Nebraska LPN license, valid driver’s license, and BLS are required. Travel to multiple locations is expected.

Qualifications

• Minimum of three years of nursing experience.
• Two years in hospice, home care, or palliative care preferred.
• Excellent written and verbal communication skills.
• Ability to work independently while coordinating with an RN and team.
• Strong documentation and care planning abilities.

Responsibilities

• Provide routine nursing visits and assess patients’ needs across physical, psychosocial, spiritual, and nutritional domains.
• Support bedside care, symptom management, crisis intervention, and family support.
• Coordinate services among hospice disciplines for each patient.
• Educate patients and families on disease process, self-care, and end-of-life care.
• Communicate with physicians, families, and team members professionally.
• Prepare accurate clinical notes, progress notes, and documentation at the point of care.
• Travel to multiple work locations to provide patient care and support hospice needs.
