Job description

The founders of Day City Hospice leveraged their extensive experience in corporate America to establish a patient-centered hospice that maximizes bedside resources while operating locally. Our mission is to provide hospice care rooted in the communities we serve, especially during the vulnerable end-of-life period for patients and their families. Our Ohio-based management team, supported by local clinicians, ensures timely and appropriate responses to patient needs.

We are committed to equipping our employees with the necessary tools to deliver exceptional hospice care in the Dayton, OH area. We maintain manageable caseloads and quotas, allowing our team more quality time with each patient. Our top priority remains our patients and those who serve them.

Schedule: Minimum of 3–12-hour shifts per month, 8a-8p

What You’ll Do:

  • Provide direct patient care as defined in the State Nurse Practice Act
  • Implement current nursing practices following comprehensive assessments and the care plan
  • Accurately and promptly document patient services to reflect the care plan
  • Assess and educate patients and families/caregivers regarding diagnosis and self-care
  • Coordinate hospice services, reporting needs to the interdisciplinary team
  • Manage pain and symptoms, evaluating patient responses to treatments and medications

Qualifications:

  • Graduate of an accredited practical nurse or vocational nursing program
  • At least three years of nursing experience; community health, hospice, or medical/surgical experience preferred
  • Current licensure as an LPN/LVN in the practicing state
  • Current CPR certification
  • Good verbal and written communication skills, organized
  • Valid driver’s license with insured vehicle according to state requirements
  • Understanding of hospice philosophy and terminally ill patient needs
