Job summary

Croí Health, a mission-driven non-profit, is seeking an experienced Hospice LPN to work 40 hours per week across the South Shore. The LPN will collaborate with the RN case manager to provide skilled nursing services in patients' homes and report changes to the physician and case manager.

Requirements include Massachusetts LPN license in good standing, 2+ years of LPN and Hospice experience, and prior homecare experience preferred.

What will you be doing:

We are looking for an experienced, talented, and compassionate Hospice LPN to work 40 hours per week throughout the South Shore.

The Hospice LPN will work collaboratively with the RN case manager to provide skilled nursing services to patients in their homes. You are responsible for the direct patient care and to provide any changes in status directly to the physician and case manager. You must work collaboratively within the team structure and have strong communication skills.

Compensation: $35-42/hr. Based on experience

What we’re looking for in you:

  • Current Massachusetts Licensed Practical Nurse license in good standing.
  • 2+ years of LPN experience required.
  • 2+ years of Hospice experience required.
  • Prior homecare and hospice experience preferred.
  • Strong clinical assessment skills and the ability to prioritize and triage patients.
  • Exceptional communication skills and the ability to work within a team structure.
  • Ability to work independently and understanding of how to access resources for patients.
