KEY JOB RESPONSIBILITIES: Provides skilled nursing care to patients in their homes under the supervision of a Registered Nurse. Provides service by each patients home health plan of care and LPNs scope of practice. Maintains compliance with federal and state regulations, reimbursement guidelines as well as agencys policies and procedures.

Additionally, the Licensed Practical Nurse (LPN) will:

  • Licensed Practical Nurse (LPN) provides skilled nursing in the home as ordered on the plan of treatment and nursing care plan.
  • Licensed Practical Nurse (LPN) demonstrates ability to continually evaluate the patients plan of care and communicate findings with Registered Nurse.
  • Licensed Practical Nurse (LPN) assist the Registered Nurse in performing specialized procedures as directed by the Registered Nurse under physicians orders.
  • Licensed Practical Nurse (LPN) evaluates the significance of assessment findings and communicates any pertinent information about the patients clinical status and ongoing needs to others in a timely fashion.
  • Licensed Practical Nurse (LPN) demonstrates the ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
  • Licensed Practical Nurse (LPN) demonstrates knowledge of medications and their correct administration based on the age of the patient and patients clinical assessment.
  • Licensed Practical Nurse (LPN) demonstrates knowledge of medications and their correct administration, follows the seven (7) medication rights to reduce the potential for medication errors. (Right: patient; medication; dose; route; time; documentation; reason)
  • Licensed Practical Nurse (LPN) performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
  • Licensed Practical Nurse (LPN) functions as a team member demonstrating collaboration with, and responsibility to patient, physician, interdisciplinary team.
  • Licensed Practical Nurse (LPN) performs patient care responsibilities considering needs to the standard of care for patients age.
  • Licensed Practical Nurse (LPN) documents wound measurements with each comprehensive assessment and weekly thereafter.
  • Licensed Practical Nurse (LPN) performs all aspects of patient care in an environment that optimizes patient safety, quality, and reduces the likelihood of medical/health care errors.
  • Licensed Practical Nurse (LPN) help optimize the capital structure and support strategic initiatives.
  • Licensed Practical Nurse (LPN) must be able to hear adequately on the telephone with no more than an amplifier and be able to communicate both verbally and in writing in English.
  • Licensed Practical Nurse (LPN) ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety, and emergency management.
  • Licensed Practical Nurse (LPN) ensures compliance with policies and procedures regarding infection prevention, control, standard precautions, and infection identification reporting.
  • Licensed Practical Nurse (LPN) always maintains patient confidentiality, including all HIPAA regulations.
  • The clinical record shall contain notes for each service provided. A clinical note will be completed for each visit made to a patients home (including a private residence, assisted living facility, group homes, or skilled nursing facility) within 48 hours of the visit.
Qualifications

  • Experience in a Hospice setting preferred.
  • Should have a minimum of one (1) year of work experience as an LPN preferable in-home care or hospital setting. Home health experience preferred.
  • Licensed LPN in the State of Indiana.
  • Knowledge of general nursing practice.

Regulatory Requirements

  • Licensed or registered in the State of Indiana.
  • Must pass a criminal background check & MVR check.
  • Completed health statements acknowledging the ability to perform the duties of the position.
  • Valid state drivers license.
  • Proof of current automobile insurance.

Licensed Practical Nurse (LPN) Hospice Expertise

  • Must be computer literate and able to maintain simple records in English.
  • Promotes patient/family education using various verbal and written communication techniques that take into account the patients/familys cultural, ethnic, and/or personal needs or preferences.
  • Collaborates with the patient/family to learn appropriate self-care techniques.
  • Submits documentation on time per agency policy.
  • Knowledgeable of current federal, state, and agency regulations concerning patient care, includes incident management and reporting.
  • Maintains regulatory requirements. Including complying with Legal Authority: Act 519, Public Acts of 1982; MCL 400.11, which states: All persons employed, licensed, registered, or certified to provide health care, must report a suspicion or reasonable cause to believe that physical, mental, or sexual abuse, neglect, or exploitation of an adult has occurred.
  • Participates in agency's other mandated activities, i.e., in-services, Quality Assurance, and Performance Improvement (QAPI); staff meetings.

EDUCATION:

  • Graduate of an accredited school of practical nursing.
  • Must hold a license to practice practical nursing in Indiana
  • Should have a minimum of one (1) year of work experience as an LPN preferable in a Hospice setting. Hospice experience preferred
  • Knowledge of general nursing practice.
