Job summary

Interim HealthCare Hospice & Palliative Care in Altoona, PA is seeking a Licensed Practical Nurse (LPN) to deliver compassionate hospice care. The role includes presenting hospice benefits, assessing discomfort, and coordinating with the RN Case Manager and Interdisciplinary Team to provide patient-centered care.

Qualifications

• Current license to practice as an LPN in Pennsylvania
• CPR/AED/BLS/First Aid certification current

• One year or more preferred hospice or similar setting experience

Responsibilities

• Present the Hospice benefit to patients and families/representatives
• Assess signs and symptoms of discomfort including pain and take prompt action
• Follow established plan of care with RN Case Manager and Interdisciplinary Team
• Communicate with patients' family to provide advice, comfort, and release instructions

As a Hospice Licensed Practical Nurse (LPN), you will:

• Present the Hospice benefit to patients and their families/representatives
• Assess signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action
• Follow established plan of care while collaborating and communicating with RN Case Manager and Interdisciplinary Team, including physician, as appropriate
• Communicate with patients' family or friends to provide advice, comfort, and release instructions.
  

To qualify for a Hospice Licensed Practical Nurse (LPN) with us, you will need:

• Licensure: Current license to practice as a Licensed Practical Nurse (LPN) in the state associated with this position
• Current CPR/AED/BLS/First Aid certification
• Reliable transportation to/from care sites and/or work locations
• Valid state-issued driver's license, automobile insurance, and reliable transportation; willingness to travel
• One (1) year preferred experience practicing as a Licensed Practical Nurse (LPN) in a hospice or similar setting
