This role supports our Hand in Hand hospice program, a part of Arc Hospice & Palliative Care family, and is dedicated to compassionate, community-rooted end-of-life care.This is a PRN (as needed) position supporting hospice patients across our service area. Assignments may vary based on patient needs and coverage requirements.
The Licensed Practical Nurse / Licensed Vocational Nurse (LPN/LVN) provides direct nursing care to patients in their homes or care settings under the supervision of a Registered Nurse. This role supports the delivery of hospice care in accordance with the plan of care, agency protocols, and the nurse’s scope of practice. The LPN/LVN performs clinical visits including routine nursing care, symptom management, and support during acute changes in condition. This team member may also be scheduled for Continuous Care assignments or assist with after-hours needs as part of the on-call rotation.
