Luminary Hospice is seeking a PRN Registered Nurse Case Manager to plan, organize, and direct hospice care in home and community settings. The role emphasizes clinical nursing experience, comprehensive assessments, and collaboration with the interdisciplinary team to meet patient needs.
The position requires current RN licensure, CPR certification, and a strong ability to educate patients and families, coordinate care, and respond to changing patient conditions.

The PRN Registered Nurse Case Manager plans, organizes, and directs hospice care and is experienced in nursing, with an emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
