As a Hospice LPN at Trinity Health At Home, you’ll deliver one-on-one, high-quality care to patients in the comfort of their homes. Using advanced technology and your clinical expertise, you’ll assess, plan, and manage individualized care that promotes healing and independence.

Minimum Qualifications
Graduate of an accredited or state approved school of practical nursing
Licensure as a LPN in the State of Michigan
A minimum of five (5) years of experience in an acute care setting; home care experience preferred.
Phlebotomy and wound care experience required.
Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.
