Job summary

Liberty Health in Goldsboro, NC is seeking an experienced Hospice Licensed Practical Nurse (LPN) to join Liberty Hospice on a full-time basis. You will provide direct patient care within the hospice program and coordinate with the RN and care team to ensure smooth service delivery.

The ideal candidate holds an approved nursing diploma, an active NC LPN license, CPR certification, and strong communication skills for patients, families, and healthcare partners.

Qualifications

• Must have graduated from an approved school of nursing.
• Current North Carolina LPN license and CPR certification.
• 1 year of clinical nursing experience, preferably in hospice or acute care.
• Ability to independently organize work and provide effective patient care with thorough documentation.

Responsibilities

• Provides direct patient care and coordinates hospice services with the RN, PCC, and other staff.
• Completes documentation as required by policy, procedure, and regulation.
• Follows the plan of care developed by the RN for each patient.
• Coordinates hospice services with the RN, PCC, and other agency personnel to optimize patient care.
• Accepts and transcribes physician orders as directed.
• Forwards all clinical, progress, and summary notes to the RN for care coordination.
• Attends and participates in IDT meetings.

Job Description

• Provides direct patient care, to include using a variety of skills and necessary coordination of services.
• Completes appropriate documentation as required by policy and procedure and regulation.
• Follows the plan of care appropriate to the patient’s diagnosis and condition as developed and assigned by the Registered Nurse.
• Coordinates Hospice services with the Registered Nurse, Patient Care Coordinator and other agency personnel to optimize patient care, as well as, the patient, family, community agencies, and physicians.
• Accepts and transcribes physician’s orders as directed.
• Forwards all clinical, progress, and summary notes to the Hospice Registered Nurse for coordination of care.
• Attends and participates in IDT meetings.

Job Requirements

• Successful completion of nursing education from an approved school of nursing.
• Current LPN licensure in the state in which the branch is located and one year of clinical nursing experience, preferably in an acute care medical/surgical setting.
• Ability to independently organize work and provide effective health care services to hospice patients.
• Ability to ensure appropriate patient care as supervised by the Hospice Registered Nurse, and document thoroughly, accurately, and completely on a timely basis.
• Effective communication skills (oral and written).
• CPR certification.
