Job summary

Mercy Health Home Care and Hospice by Compassus is seeking a Hospice Licensed Practical Nurse (LPN) to deliver direct, skilled nursing care in a home-based setting under the guidance of an RN Case Manager. You will support patients and families, build trusting relationships, and educate caregivers to ensure comfort and dignity at home.

The role emphasizes compassionate care, collaboration with the care team, and maintaining the patient’s quality of life in alignment with the plan of care.

Qualifications

• LVN license required.
• LPN license required.
