Job summary

AccentCare, Inc. is seeking a Full-Time Hospice IPC RN based in North Dade County, FL. You'll provide comprehensive nursing care, coordinate with the interdisciplinary team, and support patients and families through end-of-life care.

Training schedule is M-F 8a-5p, then 3 12s and 7p-7a with weekend rotations. Must be a licensed RN, associate degree preferred, CPR certification; hospice or home health experience preferred; be on-call, have a valid driver's license, and be able to lift up to 200

Qualifications

• Must be licensed RN; associate degree preferred; CPR certification required.
• Hospice/home health or medical-surgical experience preferred; one year in CT.
• Willingness to serve on-call and uphold mission and values.
• Valid driver's license and auto insurance required.
• Physical demands include lifting, walking, standing, and driving.
• Ability to work in varied conditions and environments.

Responsibilities

• Provide comprehensive nursing care by assessing, planning, implementing, and evaluating individualized care plans for hospice patients and their families.
• Coordinate with physicians, the interdisciplinary team, and caregivers to ensure compassionate care and proper resource use.
• Maintain accurate documentation and update patient records throughout the shift.
• Educate and support patients and caregivers on medication use, mobility, and end-of-life preparation.
• Offer emotional and physical support at time of death and participate in 24/7 care coverage with the hospice team.
• Attend team meetings, assist with supervision, training, and special projects, and adhere to nursing regulations.
