Job summary

Jobtailor invites experienced hospice RN to coordinate admissions, assess patients, and support transitions in Utah. You will collaborate with physicians and care teams to ensure timely access to hospice services and accurate documentation of care plans.

Ideal candidates hold a BSN with active RN licensure, BLS certification, and 1+ year in home health or hospice. Expect rotating days, weekends, holidays, and start times in a fast-paced environment.

Qualifications

• Current RN license in state of practice.
• Compact licenses must be transferred to the state of practice within 60 days of start date.
• Basic Life Support (BLS) certification for healthcare providers.
• BSN from an accredited institution; degree will be verified.
• RNs with <12 months experience must obtain BSN within four years of hire date.
• One year of home health or hospice experience strongly preferred.
• Experience with hospice admissions, intake, care coordination, case management, or transitions of care preferred.
• Familiarity with hospice eligibility criteria and end-of-life care services preferred.
• Candidates must reside in Utah.
• Rotating days, weekends, holidays, and start times.

Responsibilities

• Review hospice referrals.
• Conduct clinical assessments.
• Coordinate hospice admissions.
• Ensure patients receive timely access to hospice care.
• Evaluate patient needs using clinical expertise and critical thinking.
• Facilitate care transitions.
• Support patients and families during the transition to hospice care.
• Collaborate with physicians, case managers, social workers, care teams, providers, and referral sources.
• Document patient care and access clinical information.
• Operate monitors and handle complex equipment.
• Transfer, transport, and ambulate patients and distribute supplies and equipment.
