Job summary

Community Healthcare of Texas is seeking a Hospice Intake Coordinator in Cleburne, TX to manage patient admissions by coordinating referrals and the admission team nurses. You will verify referrals, coordinate timely admission information with doctors’ offices and the admission nurse, and perform routine clerical duties as needed.

Licensed Vocational Nurse (strongly preferred) with hospice intake or healthcare admissions experience is desirable to ensure accurate insurance requirements and

• Licensed Vocational Nurse (LVN) preferred for intake role.
• Healthcare intake experience required.
• Knowledge of hospice insurance requirements helpful.
• 1+ years hospice experience preferred.

Community Healthcare of Texas is currently recruiting for a Hospice Intake Coordinator.

POSITION SUMMARY

Provide patient admissions through coordination of referrals and admission team nurses.

ESSENTIAL FUNCTIONS

Receive and verify validity of patient referrals.

Coordinate with admission team pertinent information in a timely manner.

Provide admission information to doctor's office, business office, admission nurse.

Various routine clerical duties and other assignments as needed.

POSITION QUALIFICATIONS

Licensed Vocational Nurse (strongly preferred)

Healthcare intake experience required

Knowledge of insurance requirements to hospice care

1+ years of hospice experience, preferred
