Job summary

CHOT in Burleson, TX is seeking a Licensed Vocational Nurse to join our hospice team on a PRN basis. You will collaborate with an RN to assess patients, contribute to care plans, and provide compassionate end‑of‑life care.

Responsibilities include emotional support for patients and families, coordinating with CNAs, documenting care, and assisting with admissions, discharges, and crisis management as part of a dedicated, patient‑centered team.

Qualifications

• Licensed Vocational Nurse licensed by the State of Texas required.
• 1 year of nursing experience required.
• Hospice/home health experience preferred.
• Proficient with computers and MS Office.

Responsibilities

• Assess patient's physical, psycho-social, and spiritual condition with RN to plan care.
• Provide emotional support for patients and families during end-of-life care.
• Assist RN in case management, coordinate with team, and supervise CNAs.
• Document nursing care and significant interactions.
• Participate in follow-up with bereaved families.
• Assist RN with admission to Hospice House and crisis management.
• Assist RN in discharge and transfer instructions to patients/caregivers.
• Assist RN in managing multiple patients in crisis, including pharmacology.
