Essential Functions

• Delivery of Patient Care
• Coordinate and deliver patient and caregiver care.
• Conduct nursing assessments and develop/update care plans.
• Promote patient and caregiver independence.
• Utilize VIA Health Partners protocols, orders, and guidelines.
• Provide patient and family education and facilitate questions.
• Communicate effectively with the healthcare team and document care accurately.

• Participate in IDG and care conferences.
• Maintain effective team relationships and make appropriate referrals.

Qualifications

Minimum Qualifications

• Diploma or associate degree in nursing; Bachelor's preferred.

• At least 1 year of inpatient experience and 2 years in related fields such as hospice, geriatrics, or oncology.
