Essential Functions
- Delivery of Patient Care
- Coordinate and deliver patient and caregiver care.
- Conduct nursing assessments and develop/update care plans.
- Promote patient and caregiver independence.
- Utilize VIA Health Partners protocols, orders, and guidelines.
- Provide patient and family education and facilitate questions.
- Communicate effectively with the healthcare team and document care accurately.
- Interdisciplinary Group
- Participate in IDG and care conferences.
- Maintain effective team relationships and make appropriate referrals.
- Documentation
- Ensure accurate and timely clinical documentation.
- Professional and Personal Accountability
- Maintain competency, licensure, and certifications.
- Engage in professional development and uphold the organization's reputation.
- Other Duties
- Maintain cleanliness and supply stock in patient areas.
Qualifications
Minimum Qualifications
- Diploma or associate degree in nursing; Bachelor's preferred.
- Active, unrestricted RN license; multistate license preferred.
- Current CPR certification; CHPN preferred.
- At least 1 year of inpatient experience and 2 years in related fields such as hospice, geriatrics, or oncology.
- Proficiency in computer skills.
