Job summary

Agrace Homecare is seeking a dedicated Certified Nursing Assistant to join our La Crosse, WI area team on a full-time basis. The CNA will assist with activities of daily living, provide light homemaker support, and follow the care plan to ensure patient comfort and safety.

Experience with electronic health records is preferred and confidentiality is essential. Earn starting pay of $18.50/hour plus a $3,000 sign-on bonus.

Qualifications

• High school diploma or GED, or equivalent combination of education and work experience
• Graduation from an accredited Certified Nursing Assistant program
• Current, unrestricted Nursing Assistant Certification in state of employment
• One-year personal care experience, hospice, or home health, preferred
• Valid driver’s license, auto insurance, and access to a reliable vehicle
• Basic Microsoft Office skills
• Experience using electronic health records; Epic preferred
• Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property
• Ability to use proper body mechanics, safe patient handling, and standard precautions

Responsibilities

• The CNA Homecare (HC) is responsible for patient and family satisfaction by assisting with activities of daily living, providing light homemaker help, following the plan of care, communicating changes in condition, and documenting care.
• The CNA in home care (IHC) provides patient and family centered care consistent with Agrace philosophy, goals, standards, regulations and policies.
• Assist Patients with Activities of Daily Living including ambulation, toileting, bathing, eating, and post mortem care.
• Provide homemaker assistance such as light housekeeping, meal prep, and supplies management.
• Read and follow the plan of care, document in the electronic health record, and report condition changes.
• Precept new staff and students; participate in meetings to plan and evaluate services, and delegate tasks to volunteers as appropriate.

How You’ll Make a Difference (Essential Functions)

The CNA Homecare (HC) is responsible for patient and family satisfaction by assisting with activities of daily living, providing light homemaker help, following the plan of care, communicating changes in condition, and documenting care. The CNA in home care (IHC) provides patient and family centered care consistent with Agrace philosophy, goals, standards, regulations and policies.

Key Responsibilities

• Assist Patients with Activities of Daily Living
  • Assist with ambulation, changing position, and moving to/from sitting and standing.
  • Help with bathroom functions like toileting and incontinence care.
  • Help bathe, shower, dress, provide oral care, and groom.
  • Assist with eating and drinking.
  • Perform post mortem care.
  • Obtain vital signs. Assist with treatments.
  • Anticipate and respond to patient needs.
• Provide homemaker assistance
  • Perform light housekeeping such as changing linens, tidying room, doing laundry, and removing trash.
  • Order and deliver patient supplies.
  • Help prepare simple meals.
• Communication
  • Read, understand, and follow the plan of care. Document cares and changes in patient condition in electronic health record.
  • Document and report significant change in condition or unusual incidents to appropriate Interdisciplinary Team Member such as client abuse/neglect, suicidal intent, harmful coping mechanisms or violence, etc.
  • Reinforce patient, family, and facility partner education.
• Miscellaneous
  • Participate in meetings to plan and evaluate patient/family services.
  • Precept new staff and students.

You Bring (Qualifications)
