Job summary

Agrace is seeking a dedicated Certified Nursing Assistant for Homecare in the Madison area. You will provide essential daily living assistance, support for activities of daily living, and compassionate, patient-centered care in home settings.

The role requires an active CNA license, reliable transportation, and a commitment to maintaining patient dignity and comfort. Working hours are 8:00am-4:30pm, with weekend and holiday rotation as described.

Qualifications

• High school diploma or GED or equivalent combination of education and work experience.
• Graduation from an accredited Certified Nursing Assistant program.
• Current, unrestricted Nursing Assistant Certification in state of employment.
• One-year personal care experience, hospice, or home health, preferred.
• Valid driver’s license, auto insurance, and access to a reliable vehicle.
• Basic Microsoft Office skills; experience using electronic health records.

Responsibilities

• Assist Patients with Activities of Daily Living (ADLs) and mobility.
• Provide light homemaker tasks and housekeeping.
• Follow the plan of care and document care in the electronic health record.
• Read, understand, and report changes in condition to the Interdisciplinary Team.
• Participate in meetings, precept new staff, and delegate tasks to volunteers as appropriate.

Starting Pay:$18.50 + (based on experience)

How You’ll Make a Difference (Essential Functions)

The CNA Homecare (HC) is responsible for patient and family satisfaction by assisting with activities of daily living, providing light homemaker help, following the plan of care, communicating changes in condition, and documenting care. The CNA in home care (IHC) provides patient and family centered care consistent with Agrace philosophy, goals, standards, regulations and policies.

Key Responsibilities

Assist Patients with Activities of Daily Living

• Assist with ambulation, changing position, and moving to/from sitting and standing.
• Help with bathroom functions like toileting and incontinence care.
• Help bathe, shower, dress, provide oral care, and groom.
• Assist with eating and drinking.
• Perform post mortem care.
• Obtain vital signs. Assist with treatments.
• Anticipate and respond to patient needs.

Provide homemaker assistance

• Perform light housekeeping such as changing linens, tidying room, doing laundry, and removing trash.
• Order and deliver patient supplies.
• Help prepare simple meals.

Communication

• Read, understand, and follow the plan of care. Document cares and changes in patient condition in electronic health record.
• Document and report significant change in condition or unusual incidents to appropriate Interdisciplinary Team Member such as client abuse/neglect, suicidal intent, harmful coping mechanisms or violence, etc.
• Reinforce patient, family, and facility partner education.

Miscellaneous

• Participate in meetings to plan and evaluate patient/family services.
• Precept new staff and students.
• Delegate tasks to volunteers as appropriate

You Bring (Qualifications)

• High school diploma or GED, or equivalent combination of education and work experience
• Graduation from an accredited Certified Nursing Assistant program
• Current, unrestricted Nursing Assistant Certification in state of employment
• One-year personal care experience, hospice, or home health, preferred
• Valid driver’s license, auto insurance, and access to a reliable vehicle
• Basic Microsoft Office skills
• Experience using electronic health records
• Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property
• Ability to use proper body mechanics, safe patient handling, and standard precautions
