Job summary

Hackensack Meridian Health At Home is seeking a registered nurse for home health care in New Jersey. You will provide professional nursing care to patients in their homes, after hours and weekends, with a focus on 1:1 patient interaction and adherence to physician orders.

Qualified candidates have an RN license (or LPN with experience), strong communication skills, and proficiency in Office/Google Suite. A comprehensive benefits package accompanies this role.

Qualifications

• Associate's Degree in Nursing or Diploma Certificate in Nursing.
• Minimum six months RN or one year LPN experience, preferably in home care or hospice.
• Excellent written and verbal communication skills.
• Proficient with Microsoft Office and/or Google Suite.

Responsibilities

• Provide professional nursing care to home patients after hours or weekends.
• Ensure compliance with physician orders and documentation.
• Document care planning and evaluate outcomes with the care team.
• Coordinate with payors and authorization processes.
• Update care plans within two weeks of status change.
• Supervise home health aide staff and participate in evaluations.
