HOSPICE/HOME HEALTH DEPARTMENT DESCRIPTION:
Baystate Home Health and Hospice interdisciplinary care teams consist of experienced nurses, therapists, social workers, home health aides, spiritual and bereavement counselors, and volunteers. Care teams work together with both the patient and family, to ensure quality end of life care. All care is expressly tailored to meet each patients’ needs.

HHA REQUIREMENTS:
Home Health Aide Certification
Valid Driver's License
HiSet/GED or High School Diploma
POSITION RESPONSIBILITIES:
1)The Home Health Aide provides emotional and physical support to terminally ill individuals and their caregivers.
2) The Home Health Aide assists with all personal care duties in a caring and professional manner, encouraging patient independence within the patient's functional limitations.
3) The Home Health Aide assists the caregiver in teaching self care to the patient to maintain and promote the independence of individuals in a safe and healthy environment.
4) The Home Health Aide performs duties indicated in the patient care plan as outlined by clinical staff and demonstrates good judgment and skill and takes appropriate initiative to insure that patient care is of optimum quality.
