Job summary

Harmony Hospice is seeking a dedicated Home Health Aide to join our team in Rhode Island. This full-time role provides personal care and supportive services to hospice patients under the supervision of a Registered Nurse, helping ensure comfort, safety, and quality of life throughout the care journey.

The ideal candidate holds a current Rhode Island Home Health Aide license, is CPR/BLS certified, has reliable transportation, and can work independently.

Qualifications

• Must hold a current Home Health Aide license in Rhode Island.
• Must meet all state Home Health Aide training requirements, including completion of at least 75 hours of classroom and supervised practical training.
• Current BLS/CPR certification required.
• Previous hospice, home health, or patient care experience preferred.
• Understanding of the hospice philosophy and ability to provide compassionate end-of-life care.
• Ability to work independently with minimal supervision.
• Compassionate, dependable, and emotionally mature with empathy for individuals who are ill, frail, or terminally ill.
• Valid driver's license with reliable, insured transportation required.
• Excellent communication, observation, and documentation skills.

Responsibilities

• Provide personal care and activities of daily living, including bathing, grooming, dressing, toileting, oral hygiene, and mobility assistance.
• Assist patients with positioning, transfers, ambulation, and other comfort measures as directed by the Registered Nurse.
• Observe and report changes in the patient's physical, emotional, or environmental condition to the supervising nurse.
• Accurately document care provided and patient observations in accordance with hospice policies and regulatory requirements.
• Promote patient comfort, safety, dignity, and independence while following the individualized plan of care.
• Provide compassionate emotional support to patients and their families throughout the hospice journey.
• Maintain a clean, safe, and organized patient environment.
• Collaborate with the interdisciplinary hospice team to ensure continuity of care and exceptional patient outcomes.
• Perform other duties as assigned.
