Job summary

Harmony Hospice seeks a Home Health Aide to provide compassionate personal care and support to hospice patients under RN supervision. This full-time role focuses on comfort, safety and quality of life in a home setting.

Responsibilities include daily living assistance, documentation, and collaboration with the hospice team. Rhode Island HHA license and 75 hours of training required; BLS/CPR and valid driver’s license essential.

Qualifications

• Rhode Island Home Health Aide license required.
• Completion of at least 75 hours of classroom and supervised practical training.
• Current BLS/CPR certification required.
• Hospice/patient care experience preferred.
• Valid driver's license with insured transportation.
• Ability to work independently with minimal supervision.
• Compassionate, dependable and emotionally mature with empathy for patients.
• Excellent communication, observation and documentation skills.

Responsibilities

• Provide personal care and activities of daily living (bathing, grooming, dressing, toileting, oral hygiene, mobility assistance).
• Assist with positioning, transfers, ambulation and comfort measures as directed by RN.
• Observe and report changes to supervising nurse.
• Document care and patient observations per hospice policies and regulatory requirements.
• Promote comfort, safety, dignity and independence per care plan.
• Provide emotional support to patients and families throughout the hospice journey.
• Maintain a clean, safe and organized patient environment.
• Collaborate with interdisciplinary hospice team to ensure continuity of care.

The Home Health Aide provides personal care and supportive services to hospice patients under the supervision of a Registered Nurse, helping ensure comfort, safety, and quality of life throughout their care journey.

Responsibilities for Home Health Aide (Hospice):

• Provide personal care and activities of daily living, including bathing, grooming, dressing, toileting, oral hygiene, and mobility assistance.
• Assist patients with positioning, transfers, ambulation, and other comfort measures as directed by the Registered Nurse.
• Observe and report changes in the patient's physical, emotional, or environmental condition to the supervising nurse.
• Accurately document care provided and patient observations in accordance with hospice policies and regulatory requirements.
• Promote patient comfort, safety, dignity, and independence while following the individualized plan of care.
• Provide compassionate emotional support to patients and their families throughout the hospice journey.
• Maintain a clean, safe, and organized patient environment.
• Collaborate with the interdisciplinary hospice team to ensure continuity of care and exceptional patient outcomes.
• Perform other duties as assigned.

Qualifications for Home Health Aide (Hospice):

• Must hold a current Home Health Aide license in Rhode Island.
• Must meet all state Home Health Aide training requirements, including completion of at least 75 hours of classroom and supervised practical training.
• Current BLS/CPR certification required.
• Previous hospice, home health, or patient care experience preferred.
• Understanding of the hospice philosophy and ability to provide compassionate end-of-life care.
• Ability to work independently with minimal supervision.
• Compassionate, dependable, and emotionally mature with empathy for individuals who are ill, frail, or terminally ill.
• Valid driver's license with reliable, insured transportation required.
• Excellent communication, observation, and documentation skills.
