Job summary

Anchor Health in Fremont, California is seeking a compassionate Hospice Aide to support patients under RN supervision. You will perform personal care, assist with daily activities, and help promote comfort for patients in home or facility settings.

Candidates must hold both CNA and HHA licenses and have hospice experience. You will communicate with patients and families with empathy, maintain confidentiality, and report needs promptly to the Case Manager as part of the interdisciplinary team.

Qualifications

• Must have CNA and HHA licenses and hospice experience.
• Provide personal care tasks, including baths, dressing and hygiene.
• Communicate with patients and families respectfully, maintaining confidentiality.
• Attend to patient requests promptly and report needs to the Case Manager.
• Work under RN supervision as part of an interdisciplinary team.

Responsibilities

• Provide personal care including baths, back rubs, oral hygiene, shampooing and linen changes.
• Assist patients with dressing and undressing as assigned.
• Promote comfort and monitor patient needs under supervision.
• Communicate effectively with patients and families about care plans.
• Respect patient and family privacy and maintain a supportive environment.

The hospice aide is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patient's personal needs and to promote comfort.

Requirements

• 1. The Hospice Aide performs personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately.


• 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality.


• 3. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned.


• 4. The Hospice Aide assists in dressing and undressing patients as assigned.


• 5. Respectful of patient and family/caregiver environment and patient's personal needs.


• Must have both a CNA and HHA license. Must have Hospice experience.
