Dignity Hospice is seeking an RN Case Manager for full-time hospice care in Richardson, TX. You will plan, direct, and coordinate in-home hospice services, working with physicians and care teams to meet patient goals.

You will mentor Home Health Aides, maintain accurate records, and participate in discharge planning while ensuring high-quality patient care and confidentiality. On-the-road role with travel up to 75%.

Qualifications

Current Texas State license as a Registered Nurse.
2+ years’ RN experience in home health.
Reliable transportation and valid Texas Driver’s License.
Auto liability insurance current.
Must have Kinnser EMR experience.

Responsibilities

Performs comprehensive assessment for patient admission.
Develops and updates the plan of care with physician and clinicians.
Participates in discharge planning.
Provides skilled nursing care as per the plan of care.
Coordinates with other agencies to ensure continuity of care.
Engages in ongoing professional development and education.
Maintains clinical records within defined timeframes and ensures privacy.
Maintains patient confidentiality at all times.

Job Description:

As the RN case manager, you will plan, organize, and direct Hospice care for your patients. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. You will be the central point for the patient's care plan and will help lead and mentor our Home Health Aides.

Responsibilities:

Performs comprehensive assessment for patient admission
Develops and updates the plan of care in consultation with the physician and all clinicians in charge of patient's care
Participates in the patient's discharge planning process.
Provides skilled nursing care as needed in accordance with the plan of care.
Cooperates with other agencies providing nursing or related services to provide continuity of care and to implement a comprehensive care plan.
Always strives to improve nursing care by attending in-service education, through formal education, attendance at workshops, conferences, active participation in professional and related organizations.
Responsible for the overall clinical record maintenance to include visit note documentation be completed within 24 hours, comprehensive assessments, including OASIS, be completed within 48 hours, medication profile updated timely, orders/plans of care submitted timely, etc.
Maintains confidentiality of patients’ private health information.
