Job summary

Transitions LifeCare is seeking a Float LPN for full-time, field-based hospice nursing across counties in North Carolina. You will provide direct patient care under RN/IDG supervision, offer telephonic support to patients and families, and coordinate with the interdisciplinary team.

The role requires NC LPN licensure, at least 2 years of clinical experience (hospice/home health preferred), CPR, and reliable transportation.

Qualifications

• Licensed Practical Nurse with current NC license.
• Minimum 2 years of recent clinical experience (Hospice, home health, or nursing home experience preferred).
• Hospice Nurse Certification preferred.
• Knowledge of Medicare regulations, licensing laws, and accreditation standards.
• Proficient in computer skills and CPR certified.
• Reliable transportation required.
• Must be able to travel and provide care in patient homes or facilities within the service area.

Responsibilities

• Provide and document safe, competent, compassionate hospice nursing care for assigned patients under RN or physician supervision.
• Offer telephone support to patients, families, physicians, and partners, assisting with immediate needs within the LPN scope of practice.
• Review supplemental orders, perform chart audits, and assist with office duties as assigned.
• Provide end-of-life education and information to patients and families in compliance with CMS Conditions of Participation.
• Collaborate with the IDG to assess patient needs and coordinate care.
• Participate in performance improvement activities and orient/train new employees.

Education

LPN license in NC

2+ years clinical experience

Hospice Nurse Certification preferred
