Job summary

Hospice of the Piedmont is seeking a full-time LPN to join our dedicated team. Under the supervision of a registered nurse, you will deliver quality patient care and comfort measures, addressing physical, spiritual, and emotional needs.

Work hours are Monday through Friday, 8:30am to 5:00pm, with responsibilities including documenting care in the EMR, assisting physicians and RNs, administering medications, observing patient conditions, and coordinating with the interdisciplinary team.

Qualifications

• Two years’ nursing experience preferred.
• Ability to manage terminal care, health team coordination, and documentation.
• Requires intermediate word processing skills.

Responsibilities

• Deliver nursing services per Hospice policies.
• Document care using EMR.
• Assist physicians/RNs with procedures.
• Administer medications/treatments as prescribed.
• Observe, record, and report patient conditions to RN.
• Follow Plan of Care for patient/family services.
• Communicate with Interdisciplinary Team for coordinated care.
• Participate in QA programs and required meetings.
