Position Overview

In this field-based position, you will visit MJHS Hospice and Palliative Care patients to perform physical and psychosocial assessments and document visits electronically. Patient visits occur in community and facility settings. As the Hospice Field Case Manager, you will develop and implement nursing care plans and make recommendations based on patient needs.

The schedule includes working one weekend per month and serving as a backup on-call 2-3 times per month, with car and escort services provided for on-call shifts.

Qualifications

• Associates/Diploma in Nursing from an accredited school; BSN preferred.
• 2+ years’ experience in med/surg, community health, or oncology nursing preferred.
• NYS RN license.
• NYS Driver’s license.
