Job summary

MJHS in Queens, NY is seeking a Hospice Field Admissions RN to perform initial physical and psychosocial assessments of potential hospice patients. You will help develop individualized plans of care and make recommendations based on each patient’s needs, with assessments conducted in private homes, hospitals, or nursing facilities.

As a Hospice Field Admissions RN, you will admit eligible patients onto the Hospice program following NHPCO standards of practice.

Qualifications

• NYS RN license required.
• Associate degree in nursing; BSN preferred.
• 2+ years in med/surg, community health, or oncology nursing preferred.
• Must have NYS driver's license.

Responsibilities

• Perform initial physical and psychosocial assessments of potential hospice patients.
• Develop individualized plans of care, goals, and recommendations based on patient needs.
• Assessments may occur in private homes, hospitals or nursing facilities; admit eligible patients to Hospice program per NHPCO standards.
