Job summary

Home Care and Hospice is seeking a Family Support Specialist in Allegany, NY to provide direct social services to patients and families under supervision, coordinating with the hospice and home care team.

The role focuses on counseling, case management, and crisis intervention while collaborating with an interdisciplinary team; it offers a full-time schedule Monday through Friday with paid holidays and benefits.

Qualifications

• A Baccalaureate Degree in Social Work and one year of paid social work experience, or a Bachelor’s Degree in Human Services with two years of paid social work experience.
• Experience with terminally ill patients and families preferred.
• Demonstrated organizational and supervisory skills.
• Effective oral and written communication abilities.
• Valid driver’s license and automobile with current insurance.
• Availability by phone.

Responsibilities

• Establishes plans of care/objectives for patients/families in consultation with licensed social worker supervisor.
• Provides counseling and case management services to patients/families, including social, financial, emotional, and crisis intervention needs.
• Assists interdisciplinary team in understanding social and emotional factors during illness, death and bereavement.
• Participates in care plan discussions at interdisciplinary/interagency meetings.
• Participates in orientation, training and coordination of volunteers, including bereavement volunteers.
• Serves as liaison with social and community agencies to maximize effectiveness and avoid duplication.
