Job summary

Transitions Hospice is seeking an Extended Care Team (ECT) Nurse to provide adaptable, high-quality nursing care in Rockford and surrounding areas. You will reinforce the interdisciplinary team, assess patient needs quickly, and deliver skilled care during PRN visits.

Ideal candidates are experienced RNs with strong clinical judgment, excellent documentation habits, and willingness to travel locally to support care transitions and family education.

Qualifications

• RN license in the state of employment.
• Graduate from an accredited nursing program.
• Hospice/home health experience preferred.
• Valid driver’s license and ability to travel locally.

Responsibilities

• Provide nursing care across the full nursing process.
• Assess patients and families and contribute to care plans.
• Complete visit types as needed to meet patient needs.
• Administer medications and treatments as ordered by physicians.
• Document clinical notes accurately and timely.
