Visiting Nurse Home Care & Hospice in North Conway, NH seeks a Hospice Director of Patient Services to lead the clinical hospice programs and ensure compliance with federal and state regulations.

Candidates should hold MSW or RN license in NH/Maine, with 3–5 years in hospice and at least 2 years in leadership.

Job Title: Hospice Director of Patient Services

Location: North Conway, NH

Schedule: Full-time, 40 hours per week

Reports To: Executive Director

Position Summary

The Director of Patient Care Services is responsible for the overall direction of hospice clinical services. The Director of Patient Care Services establishes, implements, and evaluates goals and objectives for hospice services that meet and promote quality standards and contribute to the organization’s overall mission and philosophy.

Key Responsibilities

• Provide day-to-day operational leadership for hospice services, ensuring alignment with organizational goals, hospice philosophy, and regulatory requirements.
• Supervise, support, recruit, onboard, train, and evaluate interdisciplinary team members, including nursing, social work, chaplaincy, clinicians, and support staff.
• Ensure the safe delivery of high-quality, patient- and family-centered hospice care consistent with standards of practice.
• Monitor clinical outcomes, patient satisfaction, productivity, and quality indicators; lead quality assessment and performance improvement initiatives.
• Maintain compliance with federal, state, local, accrediting, licensure, certification, CMS Conditions of Participation, and standards.
• Develop and manage departmental budgets, staffing plans, operational indicators, inventory, supplies, equipment, and resource allocation to support fiscal responsibility and patient care needs.
• Manage agency vendors with agency leadership to ensure the delivery of goods and services to the agency’s patients. Contractual management as necessary.
• Collaborate with referral sources, community partners, internal stakeholders, and the health care community to support access to care, program growth, and hospice referrals. Work with sales/marketing team in support of the agency’s growth and development plan.
• Lead interdisciplinary team meetings and care planning processes.
• Address concerns of patients, families, and staff with professionalism and sensitivity.
• Oversee patient clinical records, statistics, reports, and documentation for evaluation and reporting of organizational activities.
• Plan and implement quarterly in-service and continuing education programs to meet staff education and training needs.
• Assist in developing organization goals, policies, and procedures that support quality hospice services.
• Collaborate with revenue cycle team members to ensure compliance and manage reimbursement risks. Participate in the preparation, response, and/or submission of any ADRs or audits.
• Stay informed about changes in clinical practices, hospice care, regulations, and reimbursement. Share relevant information with appropriate organization personnel for potential need for revisions in current agency practices or policies.

Qualifications

Current MSW or RN license in NH and Maine (RN preferred)

Minimum of 3–5 years of hospice or palliative care experience, with at least 2 years in a leadership or supervisory role.

Strong knowledge of hospice regulations, reimbursement models, and clinical standards.

Demonstrated leadership, communication, and team development skills.

Ability to communicate tactfully with customers and the community.

Experience with budgeting, financial management, and performance metrics.
